  • You (or a designated representative) AND the injured/ill employee should make the call together whenever possible.
  • Before you call, please have available:
    • Your business's federal employer identification number (FEIN)
    • Your workers' compensation policy number
    • Employee's social security number
    • Employee's personnel file
    • Any accident reports for your business
